Spotter Report Form
Used to take reports from spotters in the field. A fillable form with multiple choice items. Good for taking reports that will not be immediately sent to the NWS but will be given to them as after action items.
ICS 211SW Net Check-In form
Used to write down the names of spotters as they check in to the activation net since the net list is not used. This is a must if you are to have any chance of knowing who is out there.
ICS 309SW Communications Log
Used to record incoming communications. This can be used in place of or in addition to the Spotter Report Form. Used to record free-form text description of what is going on in the field.
